- In the body of the email please supply the name of your organization, the contact person's name and email address, and we'll send instructions on how to participate in the survey.
Prior Participants
If you participated in the 2012 survey, please log into the survey at https://surveys.ihstrategies.com, using the same username and password provided to you in a previous communication from INTEGRATED Healthcare Strategies. Once logged in, access the survey instrument by selecting "Upload and Download Your Questionnaires" from the list displayed. Your questionnaires will be pre-populated with the data you submitted before. Also, if you are responsible for reporting data for more than one organization or subsidiary, you will be able to report data for ALL of these organizations in one instrument.
